We enjoy meeting new people and welcome new patients to our clinic. For your first visit to our office we recommend a complete New Patient Exam, plus any necessary x-rays. During this exam your dentist will review your dental and medical history, examine your teeth, gums, and surrounding areas. Your dentist will recommend any necessary dental treatments and may suggest dental cleanings with our highly qualified dental hygienists. We will work with you to set up a schedule for future appointments - and we will consider your health, your lifestyle, and your budget to come up with treatment plan options that are right for you. We require patient information to help us understand your needs and your current dental and medical health. To help facilitate your first appointment, we have provided a New Patient Information chart you can fill out and submit prior to your first appointment at our office. Secure and confidential, another service to make your dental care even easier!
X-rays are a valuable diagnostic tool; if you had x-rays taken at another dental facility within the past two years these may be useful and save us from repeating the process. Many offices will forward recent x-rays to us; we have provided an X-ray Requst Form for you to simplify this.
Dental insurance information is necessary in order to bill your insurance plan. The following information is required: Insurance Provider name, Group or Plan number, ID or certificate number. If you have coverage but do not have the information available at your appointment time, we will collect payment from you and provide you with a completed claimform to submit to your plan for reimbursement.
Understanding your dental coverage is important to help plan your treatment and appointments. Your insurance provider may have given you a benefit information booket, which you can bring along to your first appointment. If not, we have provided a downloadable form to help Understand Dental Insurance coverage. Your insurance company should be able to provide the coverage and frequency information by the procedure name and code number, either online or by telephone. This will help both you and our office to understand how to best make your plan work for you.
